You must submit a claim form by December 31, 2011 if you want to take part in the Membership Program Debit Card Litigation Class Action Lawsuit Settlement at www.DebitCardSettlement.com. If you do not want to take part in the settlement and you wan to sue the companies on your own, then you will need to exclude yourself. The deadline to do this is August 8, 2011. If you want to stay in the settlement but you want to object, you will need to submit your request by August 8, 2011. The Fairness Hearing is September 16, 2011.
